However we have provided some options for the patient in order to be more accessible pricewise to those who would like to take advantage of our services. Please keep in mind that though our pricing options are flexible based on time, we strive to provide the utmost quality of care and service first and foremost. For this reason, the final decision for length of appointment time remains with the treating practitioner. New Patient Enrollment For your convenience, we have made a number of forms available for new and existing patients to download and complete prior to office visit. If you are a New Patient, please print and complete the below forms and fax prior to your appointment to 404-478-9869. Existing patients, please print and complete the appropriate form/s and fax or bring with you to our office on the date of your scheduled appointment.
